Welcome To Our Neighborhood!

Thank you for considering the Hillsdale County Medical Care Facility and Rehabilitation Center as your choice for rehabilitative services or long-term skilled nursing care. Your County Medical Care Facility is staffed with State of Michigan licensed and certified caregivers whose primary focus is providing the best quality and comprehensive care possible. Our local community physicians pride themselves on providing continuing care to their patients and seeing them personally on a regular basis. Residents don’t live in our facility; we work in their home. A resident-centered plan of care that is designed around one’s personal likes, dislikes, and preferences is standard. Our updated facility consists of 130 private rooms and 20 semi-private rooms with large accessible living rooms, reading rooms, and fully equipped dining rooms. Large common areas are available for private family functions and facility activities. Outdoor activities are important and our facility boasts two expansive, secure outdoor courtyards, including everything you will need for private family picnics and facility-planned events. Our goal is to make your stay in our neighborhoods as homelike as possible. We understand that whether you or your loved one’s stay is rehabilitative or long-term, it is not an easy decision. 
With today’s financial climate and increasing insurance options, choosing the appropriate medical care is a difficult decision and process. Our admission team is continuously updated on the ever changing world of rehabilitative and long term care insurance. We are ready to answer any insurance questions you may have. We regularly work with Medicare, Medicare supplemental insurance, Michigan Medicaid, and all major insurance companies to verify and maximize your insurance benefits.
Hillsdale County Medical Care Facility accepts Michigan Medicaid and our admission staff will walk you and your loved one through the process, answer your questions, and help you complete all of the necessary forms and applications. 

If you would like to schedule a tour, have any questions, or would like to be contacted by our admissions staff, please complete and submit the Admission Questionnaire or telephone the Admissions Department at 517-439-2842. Please feel free to download and print the HCMCF Admission Application below. You may complete the application and bring it along to your appointment if you choose.

  • Physical Therapy
    Physical therapy can help patients regain their mobility and independence after suffering from orthopedic or neurological disorders. The utilization of cutting-edge methods helps patients regain their full range of motion while also reducing their level of discomfort. You may get closer to your functional goals by increasing your range of motion strength, coordination, and muscular control.
  • Occupational Therapy
    Occupational therapy aims to help residents regain their ability to do everyday tasks such as washing, dressing, and eating after a medical or physical condition. Our therapists employ rehabilitation approaches and teach residents how to use specialized equipment in order to maximize function and mobility.
  • Speech Therapy
    In speech-language therapy, the resident's ability to express themselves verbally and solve problems, read, write, and articulate their thoughts more clearly is improved. Treatment for swallowing problems is also provided by therapists.
